1. FMEA vs FMECA: Foundations

Developed by the US Military (MIL-STD-1629A) and adopted worldwide by automotive (AIAG-VDA) and aerospace (SAE ARP5580) industries, Failure Mode and Effects Analysis (FMEA) is a bottom-up, inductive methodology for proactively identifying potential failure modes before commissioning.

When quantitative failure rates and consequence probabilities are coupled to the analysis, it becomes FMECA (Failure Mode, Effects, and Criticality Analysis).

2. The Fatal Flaw of Legacy RPN (Risk Priority Number)

For decades, engineers evaluated risk via the simple product:

RPN = Severity (S) × Occurrence (O) × Detection (D)

Modern safety engineers recognized critical mathematical and operational flaws in this legacy metric:

  • False Equivalency: A failure mode with S=10, O=1, D=1 (RPN=10) presents catastrophic safety risk (loss of life), while a mode with S=1, O=5, D=4 (RPN=20) is trivial cosmetic scrap. Blindly sorting by RPN causes teams to ignore lethal hazards.
  • Arbitrary Thresholds: Arbitrary corporate policies stating "mitigate all RPN > 100" incentivize teams to artificially deflate Detection ratings to 99 to avoid corrective paperwork.

3. AIAG-VDA Action Priority (AP) Framework

In 2019, the joint AIAG & VDA FMEA standard replaced RPN with Action Priority (AP). AP categorizes items into High (H), Medium (M), and Low (L) priority using multi-dimensional decision tables.

Under AP logic, any failure mode with Severity 9 or 10 receives High priority regardless of Occurrence or Detection unless occurrence is provably near-zero and detection is fully automated fail-safe interlock.
